Position Summary

The APS Case Manager is responsible for delivering short‑term, client‑centered care coordination and supportive services to APS clients with complex medical, psychosocial, functional, and housing‑related needs. The position emphasizes risk mitigation, service coordination, and housing stabilization, while respecting client autonomy, cultural preferences, and legal rights.

The APS Case Manager works collaboratively with APS Investigators, multidisciplinary teams, community partners, and service providers to promote safety, reduce risk, and support clients’ ability to remain safely housed in the least restrictive environment.

Essential Duties and Responsibilities

  • Supports and promotes the mission of the Agency: Foothill Family empowers children and families on their journey to achieve personal success.
  • Conducts comprehensive bio‑psychosocial, functional, and environmental assessments for APS clients referred to the ICS Program.
  • Develops and implements short‑term, individualized care plans that address safety, health, housing stability, and functional needs.
  • Provides care coordination and supportive services under APS Baseline (AB 135), including referrals to medical, behavioral health, in‑home support, nutrition, transportation, and social services.
  • Provides housing stabilization services under Home Safe (SB 101), which may include housing navigation, landlord coordination, financial assistance, and linkage to permanent housing resources.
  • Collaborates closely with APS Investigators to support ongoing abuse investigations and risk reduction efforts.
  • Coordinates with community partners, service providers, hospitals, shelters, housing agencies, and legal entities to ensure timely service delivery.
  • Drives as an essential job function throughout Los Angeles County to conduct home visits and fieldwork, ensuring timely monitoring of client safety and evaluation of service effectiveness.
  • Documents all assessments, service plans, contacts, and outcomes accurately and timely in the APS case management system in compliance with County, State, and funding requirements.
  • Participates in multidisciplinary team meetings, case conferences, and program reviews.
  • Supports client self‑determination while balancing mandated reporter responsibilities and APS statutory requirements.
  • Prepares written reports and documentation as required for audits, funding compliance, or court proceedings.
  • Provides services relating to the administration of public funds or benefits, including eligibility for public funds or public benefits.
  • Displays sensitivity to the service population’s cultural and socioeconomics characteristics.
  • Performs work in a safe manner at all times and ensures that other individuals do not put themselves or others at risk.
  • Follows all policies, procedures and legal mandates related to HIPAA and maintenance of client files.
  • Ensures performance and behaviors support the Agency’s performance quality improvement (PQI) goals and objectives.
  • Reports to work on time and maintains reliable and regular attendance.
  • Performs other duties as assigned.

Position Requirements

  • Bachelor’s degree in social work, human services, psychology, sociology, gerontology, or a closely related field.
  • Five (5) years of professional experience in case management, protective services, social services, healthcare, or housing services.
  • Knowledge of Adult Protective Services laws, elder/dependent adult abuse indicators, and service delivery systems.
  • Knowledge of California APS statutes, mandated reporting requirements, and ethical practice standards.
  • Ability to assess safety, capacity, and risk in complex, high‑needs situations.
  • Strong care coordination, problem‑solving, and crisis‑intervention skills.
  • Ability to work effectively with individuals from diverse cultural, socioeconomic, and linguistic backgrounds.
  • Requires regular and reliable driving throughout Los Angeles County.
  • Valid CA Driver’s License and maintains insurability on the Agency’s auto liability policy (including a minimum of 2 years driving experience) and maintains the California state-required auto insurance liability limits

Preferred Qualifications

  • Master’s degree in social work or a related field.
  • Prior experience in Adult Protective Services, Home Safe, homelessness services, or integrated care coordination programs.
  • Experience working with older adults, dependent adults, or unhoused populations.
  • Bilingual consistent with community needs (e.g., Spanish, Cantonese, Mandarin, Korean, Vietnamese, Armenian).
  • Strong written and verbal communication skills, including documentation for compliance and audits.
  • Ability to manage a caseload and meet established timelines and performance benchmarks.
