Bethesda Health is seeking a dedicated LPN Rehab Nurse for a Full-Time 12-hour Night Shift position within our Healthcare & Medical Services department. In this role, you will provide high-quality, compassionate nursing care to patients undergoing rehabilitation for medical, surgical, orthopedic, and neurologic conditions. Working closely with RNs, therapists, physicians, and the broader interdisciplinary team, you will support individualized care plans designed to promote recovery, functional independence, and safe transitions of care. As an LPN Rehab Nurse, you will perform direct patient care including medication administration, treatments, wound and skin care, assistance with mobility and activities of daily living, pain management, and monitoring of vital signs and clinical status throughout the night. You will accurately document assessments, interventions, and patient responses in the electronic health record, communicate changes in condition promptly to the RN and provider, and actively participate in bedside reporting and care conferences to support continuity of care. You will play a key role in patient and family education, reinforcing therapy goals, safety measures, and discharge instructions to help patients regain independence and prevent complications or readmissions. This includes safe use of assistive devices, fall prevention, skin integrity preservation, and adherence to medication and therapy regimens. You will collaborate with physical, occupational, and speech therapists to support rehab goals, assist with transfers and ambulation, and encourage adherence to treatment plans while maintaining patient dignity and comfort.
